loudness is measured to ITU-R BS.1770-4 and matches pyloudnorm within 0.05 LU. true peak matches ffmpeg within 0.01 dB. the limiter aims 0.15 dB under the stated ceiling, because independent meters routinely disagree by around 0.1 dB and the file should measure compliant on someone else's meter too.
source sample rate is preserved. output is wav, 24 or 16 bit. changing any setting re-runs everything from the decoded audio, so there's no need to load the files again.
